Care Coordinator / Administrator
Location: North West Kent
Salary: £25,000 – £27,000 (dependent on experience)

We are recruiting for a Care Coordinator / Administrator on behalf of Primary Care Network (PCN,) based in North West Kent on a full time basis, Monday-Friday 09:00-17:00.
About the Role
Care Coordinators play a vital role in supporting patients to navigate health and care services, ensuring coordinated, proactive and personalised care. This position sits at the heart of the PCN, working closely with GPs, allied health professionals, social prescribing link workers and wider community services to improve patient outcomes and quality of life.

Key Responsibilities

Coordinate and manage appointments across primary, secondary, community, voluntary and statutory services

Support population health management through risk stratification, data analysis and patient lists

Work closely with social prescribing link workers and the multidisciplinary team to ensure joined-up care

Act as a central point of contact for patients, supporting them before and after clinical conversations

Escalate clinical concerns, safeguarding issues or unmet needs to appropriate clinicians

Maintain accurate patient records using EMIS Web, EMIS Enterprise and other systems

Provide administrative support to PCN services, projects, meetings and events

Contribute to service monitoring, reporting and continuous improvement initiatives
About You

GCSEs (A–C) in English and Maths

Experience working in a multi-professional environment

Strong IT skills, including Microsoft Excel (EMIS experience desirable)

Excellent communication and organisational skills

Caring, proactive and person-focused
